Certificate of Insurance
Most cities and towns require proof of insurance to allow teams to play on their fields. Teams can self issue insurance requests and access current COIs via the league's insurance provider site. These permits are only valid for BMBO games.

How to Request a New COI
If your town is not listed, here are directions on how to self issue the request and get your COI in 60 seconds.
Scroll to the bottom of the page and click on "Self issue COI"
- Provide name and mailing address of entity requesting to be named on the certificate
- Indicate Property Owner/Lessor
- Click "no" to the question about additional wording/endorsements (unless specifically requested by property owner)
- Click "add certificate"
